The Generational Differences in PTSD Treatment

October 2016 - Post-Traumatic Stress Disorder (PTSD) is one of the most misunderstood mental health conditions, and there is a wealth of myths and mistruths that surround the condition. Many people, for example, believe that anyone who is exposed to trauma is given a diagnosis of Post-traumatic stress disorder, whilst others don't believe that the condition exists at all.
In reality, PTSD is a long term psychological response to a life threatening experience or other significant trauma. Anyone can experience PTSD, although it is most common amongst returning veterans that have experienced combat, victims of rape or abuse, or those who have witnessed traumatic events such as a car accident, murder, or other significant incident. The symptoms of PTSD include suffering from flashbacks of the event, intrusive and distressing thoughts that you are unable to control, experiencing nightmares, emotional numbness and hyper-exaggerated responses to perceived danger. An estimated 8% of the American population will experience PTSD at some point in their lives, and PTSD affects approximately 8 million adults in any given year. The largest part of the American population to suffer from PTSD is the military population, particularly returning veterans. One in three returning troops are diagnosed with serious post-traumatic stress symptoms and go on to develop the condition. However a marked difference occurs in the treatment and diagnosis of PTSD amongst returning servicemen depending on their generation, which can prove fascinating.
A Question of Acceptance and Awareness
Current military veterans are often referred to as 'Generation PTSD' because of the high number of returning veterans that are presenting with the condition, when compared to the returning veterans of other wars. However that doesn't mean that PTSD occurs more in veterans of today's wars in Iraq and Afghanistan than it did amongst veterans of Vietnam and the Gulf War: simply that awareness means more veterans are receiving the diagnosis and treatment that they need. Vietnam war veterans, for example, are often known as the forgotten generation and whilst with hindsight we can see that many veterans presented with the symptoms of PTSD, they were often not treated and chose instead to self-medicate with drugs or alcohol. More than 200,000 Vietnam veterans are still experiencing PTSD today, and yet many of those veterans were not offered treatment for their conditions until decades after they had returned from war. The prevailing attitude was to brush the war under the carpet once you had returned: not to talk about it, not to acknowledge your trauma, and certainly not to admit that you needed any help or support.
What's more, research conducted by Dr. Isabelle Mansuy and published in the Biological Psychiatry journal found that PTSD is more common amongst so-called 'second generation' survivors of trauma than amongst the general population. So, for example, if a father returns from Vietnam with PTSD and then his son decides to enlist and is sent to Iraq then that son is statistically more likely to suffer from PTSD than his colleague with no family history of significant trauma or PTSD.
Finding the Right Support Network
The good news is that PTSD does not last forever, and with the right treatment and support network the symptoms of the condition will begin to resolve themselves. However with a significant number of veterans choosing not to seek treatment and support for their condition, simply PTSD can become what is known as 'complex PTSD'; a condition that is more long term and more difficult to treat. That is why it is so important that those individuals in situations where known PTSD triggers are present (such as individuals in war zones or experiencing close combat) are closely monitored and supported as soon as any of the symptoms of PTSD begin to present. There is a rising suicide rate amongst veterans, and this is largely attributed to PTSD (both diagnosed and undiagnosed) so it is critical that veterans are given as much help and support as possible. The support of both their families and of the Veterans Association (VA) is essential in the battle to reverse this trend and help put struggling veterans back on the right path once more.

स्त्रियों की समस्या

  मैं आज स्रियों की समस्या के विषय में कुछ कहना चाहती हूं । यह समस्या देखने - में तो उतनी ही पुरानी है जितनी की मनुष्यजाति, परंतु अपने मूत्र में यह इससे भी ' बहुत अधिक पुरानी हैं। कारण, यदि कोई एक ऐसे नियम को ढूंढना चाहे जो इसका शासन तथा समाधान करता हैं , तो उसे विश्व के उद्गम तक, बल्कि सृष्टि के भी परे जाना होगा ।

  कुछ प्राचीनतर परंपराएं, संभवत: प्राचीनतम परंपराएं विश्व की उत्पत्ति का कारण 'सर्वोच्च सत्ता' का वह संकल्प बताती हैं जो आत्मनिष्ठ रूप में अपने-आपको व्यक्त करने के लिये होता हैं। ऐसा प्रतीत होता हैं कि इस विषयीकरण का पहला कार्य था सृजनात्मक 'चेतना' का प्रकट होना । यह सत्य है कि ये प्राचीन परंपराएं अभ्यासवश ही 'सर्वोच्च सत्ता' की पुल्लिंग के रूप में और 'चेतना' की सीलिंग के रूप में चर्चा करती हैं तथा इस आदि भाव को ही पुरुष और स्त्री के विभेद का स्रोत बना देती हैं, इसी के दुरा वे पुरुष को स्त्री पर प्रधानता भी दे देती हैं, जब कि वास्तविक बात यह हैं कि अभिव्यक्ति सें पहले दोनों हीं एक, अभिद्रव तथा सहवर्ती थे । पुरुष-सत्ता ने हीं पहला निर्णय किया और उसी ने उस निर्णय को चरितार्थ करने के लिये सर्वसत्ता को जन्म भी दिया । इसका अर्थ यह हुआ कि यदि स्त्री-सत्ता के बिना सृष्टि-कार्य नहीं हो सकता, तो पुरुष-सत्ता के प्रारंभिक निश्चय के बिना स्त्री-सत्ता का आविर्भाव भी नहीं हो सकता ।

  निश्चय ही, यहां यह प्रश्र किया जा सकता हैं कि क्या यह व्याख्या कुछ अत्यधिक मानवीय नहीं हैं । किंतु, सच्ची बात यह है कि समस्त व्याख्याएं ही, जो कि मनुष्य कर सकता हैं, कम-से-कम अपने बाह्य स्वरूप में, अवश्य हीं मानवीय होगी । कारण, कुछ असाधारण व्यक्ति उस ' अज्ञेय' ओर ' अचिंत्य' की ओर अपनी आध्यात्मिक चढ़ाई में मानव प्रकृति से ऊपर जा सके हैं तथा अपनी खोज के ध्येय के साथ, एक उच्च तथा एक प्रकार की अकल्पनीय अनुभूति में, एक हों सके हैं, किंतु ज्यों हीं उन्होंने अपनी उपलब्धि से दूसरों को लाभ पहुंचाना चाहा, उन्हें उसे सूत्रबद्ध करना पड़ा और उनके सूत्र को तब ग्राह्य बनने के लिये मानवीय और प्रतीकात्मक होना पड़ा । फिर भी, यह प्रश्र किया जा सकता है कि क्या ये अनुभव और इनके दुरा प्रदर्शित सत्य प्रधानता के उस भाव के लिये उत्तरदायी हैं जो पुरुष खिल के प्रति हमेशा बनाये रखता है, या, इसके विपरीत, सामान्य रूप से प्रचलित यह प्रधानता का भाव हीं अनुभूतियों के सूत्रबद्ध रूप के लिये उत्तरदायी हैं...

  बहरहाल, यह तथ्य तो निर्विवाद ही है कि पुरुष अपने-आपको बढ़ा सम्मतता है तथा अपना प्रभुत्व जमाना चाहता हैं, उधर स्त्री अपने-आपको उत्पीड़ित अनुभव करती हैं और फिर परोक्ष या अपरोक्ष रूप में विद्रोह करती है; और इन दोनों का यह झगड़ा

युग-यग सें चला आ रहा हो यह स्व मे एक हीं हैं, पर अनगिनत रूप-रंगों मे प्रकट होता हैं ।

  यह तो मानो हुई बात हैं कि पुरुष सारा दोष स्त्री पर थोपता है और उसी प्रकार स्त्री सारा दोष पुरुष पर थोपती हैं । पर, वास्तव में, दोष समान रूप में दोनों का मानना चाहिये और दोनों में से किसी को भी अपने-आपको दूसरों से बढ़ा मानने का गर्व नहीं करना चाहिये । बल्कि जबतक प्रधानता और हीनता का यह विचार छू नहीं कर दिया जायेगा तबतक कोई भी वस्तु या कई भी व्यक्ति इस भ्रांति को दूर नहीं कर सकेगा जो मानवजाति को दो विरोधी शिविरों मे बांट देती है, और न तबतक समस्या का कोई समाधान हीं हो पायेगा ।

  इस समस्या पर बहुत कुछ कहा और लिखा गया है । इस पर इतने दृष्टिकोण से विचार किया गया है कि इसके सब पक्षों का विवेचन करने के लिये एक पोथा भी पर्याप्त न होगा । साधारणतया, सिद्धांत बहुत अच्छे होते हैं और हर एक के अपने- अपने गुण भी होते हैं, किंतु व्यवहार में ये उतने सुखदायी नहीं सिद्ध होते । मुझे नहीं मालूम कि सफलता के स्तर पर हम पाषाण-युग से कुछ आगे बढ़े हैं या नहीं । कारण, पारस्परिक संबंध मे पुरुष और स्त्री एक दूसरे के पूरी तरह निरंकुश स्वामी और साथ हीं कुछ दयनीय दास भी होते हैं ।

  हां, सचमुच दास; क्योंकि जबतक मनुष्य में इच्छाएं हैं, अभिरुचि और आसक्तिया हैं, तबतक वह इन वस्तुओं का और उन व्यक्तियों का भी दास बे जिन पर वह इन इच्छाओं की पूर्ति के लिये निर्भर रहता है ।

  अतएव, स्त्री पुरुष की दासी इसलिये हैं कि वह पुरुष और उसके बल के प्रति आकर्षण अनुभव करती हैं , उसके अंदर घर बसाने की इच्छा होती हैं, वह घर से प्राप्त होनेवाली सुरक्षा को चाहती हैं, और अंत में उसके अंदर मातृत्व के प्रति मोह भी होता है । इधर पुरुष भी स्त्री का दास हैं , अधिकार-भावना के कारण, शक्ति और प्रभुत्व की तृष्णा के कारण, काम-वासना की तृप्ति की इच्छा तथा विवाहित जीवन की छोटी- मोटी सुख-सुविधाओं के प्रति आसक्ति के कारण ।

  इसलिये कोई भी कानून स्त्री को तबतक बंधनमुक्त्त नहीं कर सकता जबतक वह स्वयं हीं बंधनमुक्त्त न हो जाये; इसी प्रकार पुरुष भी अधिकार जमाने की आदतों के होते हुए तबतक दासता से मुक्त नहीं हो सकता जबतक वह अपने अंदर की सारी दासता से मुक्त न हो जाये ।

  यह गुप्त संघर्ष की अवस्था जिसे प्रायः कोई स्वीकार नहीं करता; किंतु जो, अच्छे-सें-अच्छे दृष्टांत मे भी, सदा अवचेतन में उपस्थित रहती है, तबतक अनिवार्य प्रतीत होती हैं, जबतक मनुष्य पूर्ण चेतना के साथ तादात्म्य स्थापित करने के लिये, 'सर्वोच्च सत्ता' के साथ एक होने के लिये, अपनी सामान्य चेतना से ऊपर नहीं उठ जाते । कारण, जब तुम उस उच्च चेतना को प्राप्त कर लेते हो तो देखते हो कि

पुरुष और स्त्री का भेद केवल शारीरिक भेद रह जाता हैं ।

  हो सकता हैं कि वस्तुतः, प्रारंभ में पृथ्वी पर एक विशुद्ध पुल्लिंग और एक विशुद्ध सीलिंग का प्रतिरूप रहा हो, प्रत्येक के अपने-अपने स्पष्ट भिन्न प्रकार के गुण रहे होंगे, किंतु समय के प्रवाह में अनिवार्य मिश्रण, आनुवंशिकता, पुत्रों का माता से सादृश्य और पुत्रियों का पिता से सादृश्य, सामाजिक उन्नति, एक ही व्यवसाय-इन सबने मिलकर हमारे समय में एक विशुद्ध प्रतिरूप को पाना दुर्लभ कर दिया हैं । सब पुरुष अपने कई पक्षों में स्त्री-सदृश हैं । इसी प्रकार सब स्त्रियों भी कई गुणों के ख्याल से, विशेषतया आधुनिक समाज में, पुरुष-सदृश हैं । दुर्भाग्य से, शारीरिक आकृति के कारण झगड़े की आदत चली आ रही हैं, बल्कि प्रतिदुंदिता की भावना के कारण शायद बढ़ भी गयी है ।

  पुरुष और स्त्री , दोनों हीं अपने अच्छे क्षणों में लिंग-भेद मूल जाते हैं, किंतु जरा-सी उत्तेजना पाते हीं वह भेद फिर से आ जाता है; स्त्री अनुभव करने लगती हैं कि वह स्त्री है, और पुरुष तो यह जानता ही हैं कि वह पुरुष है और झगड़ा फिर अशिक्षित अवधि के लिये, किसी-न-किसी रूप में, प्रत्यक्ष या परोक्ष स्तर पर चलने लगता है और प्रकट रूप में जितना कम स्वीकार किया जाता है उतना हीं कटु होता है । कोई पूछ सकता है कि क्या यह झगड़ा तबतक ऐसा ही न चलता रहेगा जबतक पुरुष और स्त्री न रहकर ऐसी जीवंत आत्माएं नहीं बन जाते जो लिंग-रहित शरीर में अपने एक ही अमित्र स्रोत को अभिव्यक्त करती हों ।

  कारण, हम एक ऐसे संसार का स्वप्न देखते हैं जिसमें अंततः, ये सब विरोध विलीन हों जायेंगे, जहां केवल एक ऐसी सत्ता हीं जीवित रह सकेगी तथा उन्नति को प्राप्त होगी जो उस सबका, जो मानव सृष्टि मे सर्वश्रेष्ठ है, सामंजस्यपूर्ण समन्वय होगी और जो अखंड चेतना एवं क्रिया मे, विचार एवं कार्यान्वित में, अंतर्दृष्टि एवं सृजन मे एकत्व लाभ कर लेगी ।

  जबतक समस्या का यह सुखद और आमूल समाधान नहीं हो जाता, भारतवर्ष और बातों की भांति इस बात में भी उन प्रचंड विरोधात्मक भेदों का देश रहेगा जिन्हें फिर भी एक अत्यंत व्यापक एवं विस्तृत समन्वय में परिणत किया जा सकता है ।

  वस्तुतः, क्या भारतवर्ष मे ही उस परम जननी की अत्यधिक तीव्र भक्ति और पूर्ण उपासना नहीं की जाती जो विश्व को बतानेवाली और शत्रुओं पर विजय पानेवाली हैं, जो समस्त देवताओं और समस्त जगतों की माता है, सकल-वरदायिनी है?

   और क्या भारत में ही हम स्त्री -तत्त्व, 'प्रकृति', अर्थात् 'माया' की अत्यंत आमूल रूप में निंदा और उसके प्रति अत्यधिक घृणा प्रदर्शित होते नहीं देखते, क्योंकि वह एक विकारजनक भ्रम हैं तथा समस्त दुःख और पतन का कारण हैं, अर्थात् ऐसी प्रकृति हैं जो विमोहित और कलुषित करती हैं तथा व्यक्ति को भगवान् सें ले जाती है?

भारतवर्ष का सारा जीवन हीं इस विरोध से सराबोर हैं; वह अपने मन और हृदय, दोनों में इससे पीड़ित हैं । यहां, सर्वत्र, मंदिरों में देवियों की मूर्तिया प्रतिष्ठित हैं, मां दुर्गा हैं हीं भारतवर्ष की संतानें मुक्ति और मोक्ष की आशा करती हैं । और फिर भी एक भारतवासी ने ही यह कहा है कि अवतार कभी स्त्री के शरीर में जन्म नहीं लेगा, क्योंकि तब कोई विचारवान हिंदू उसे न पहचान पायेगा! पर यह प्रसन्नता की बात हैं कि भगवान् इस संकीर्ण सांप्रदायिक भावना सें प्रभावित नहीं होते और न ही इन तुच्छ विचारों द्वारा प्रेरित होते हैं । जब उनकी पार्थिव शरीर में अवतरित होने की इच्छा होती है तो वह इस बात की परवाह कम ही करते हैं कि लोग उन्हें पहचानेंगे या नहीं । इसके अतिरिक्त, ऐसा प्रतीत होता है कि अपने सब अवतारों में उन्होंने विद्वानों की अपेक्षा बच्चों और सरल हृदयों को अधिक पसंद किया हैं।

  जो भी हों, जबतक एक ऐसी नयी जाति को, जिसे प्रजनन की आवश्यकता के अधीन होने की जरूरत न हो और जो सत्ता के दो पूरक लिंगों में विभाजित होने के लिये बाध्य न हों, उत्पन्न करने के लिये प्रकृति को प्रेरित करनेवाला नया विचार एवं नयी चेतना प्रकट नहीं हो जाते, तबतक वर्तमान मानवजाति की उन्नति के लिये अधिक-सें-अधिक यहीं किया जा सकता हैं कि पुरुष और स्त्री दोनों के साथ पूर्ण समानता का व्यवहार किया जाये, दोनों को एक ही शिक्षा तथा प्रशिक्षा दी जाये तथा दिव्य सत्ता के साथ, जो कि समस्त लिंग-भेदों सें ऊपर हैं, सतत संपर्क स्थापित करके समस्त संभावनाओं और समस्त समस्वरताओं के उद्गम को प्राप्त किया जाये ।

 और तब शायद भारतवर्ष, जो विषमताओं का देश हैं, नयी उपलब्धियों का देश बन जायेगा, जैसे यह इनकी परिकल्पना का पालना रहा हैं।

The Year of the MOOC

IN late September, as workers applied joint compound to new office walls, hoodie-clad

colleagues who had just met were working together on deadline. Film editors, code-writing

interns and edX fellows grad students and postdocs versed in online education were

translating videotaped lectures into MOOCs, or massive open online courses. As if anyone

needed reminding, a row of aqua Post-its gave the dates the courses would go live.

The paint is barely dry, yet edX, the nonprofit start-up from Harvard and the Massachusetts

Institute of Technology, has 370,000 students this fall in its first official courses. Thats nothing.

Coursera, founded just last January, has reached more than 1.7 million growing faster than

Facebook, boasts Andrew Ng, on leave from Stanford to run his for-profit MOOC provider.

This has caught all of us by surprise, says David Stavens, who formed a company called

Udacity with Sebastian Thrun and Michael Sokolsky after more than 150,000 signed up for Dr.

Thruns Introduction to Artificial Intelligence last fall, starting the revolution that has higher

education gasping. A year ago, he marvels, we were three guys in Sebastians living room and

now we have 40 employees full time.

I like to call this the year of disruption, says Anant Agarwal, president of edX, and the year is

not over yet.

MOOCs have been around for a few years as collaborative techie learning events, but this is the

year everyone wants in. Elite universities are partnering with Coursera at a furious pace. It now

offers courses from 33 of the biggest names in postsecondary education, including Princeton,

Brown, Columbia and Duke. In September, Google unleashed a MOOC-building online tool, and

Stanford unveiled Class2Go with two courses.

Nick McKeown is teaching one of them, on computer networking, with Philip Levis (the one

with a shock of magenta hair in the introductory video). Dr. McKeown sums up the energy of

this grand experiment when he gushes, Were both very excited. Casually draped over

auditorium seats, the professors also acknowledge that they are not exactly sure how this

MOOC stuff works.

We are just going to see how this goes over the next few weeks, says Dr. McKeown.

WHAT IS A MOOC ANYWAY?

Traditional online courses charge tuition, carry credit and limit enrollment to a few dozen to

ensure interaction with instructors. The MOOC, on the other hand, is usually free, credit-less

and, well, massive.

Because anyone with an Internet connection can enroll, faculty cant possibly respond to

students individually. So the course design how material is presented and the interactivity

counts for a lot. As do fellow students. Classmates may lean on one another in study groups

organized in their towns, in online forums or, the prickly part, for grading work.

The evolving form knits together education, entertainment (think gaming) and social

networking. Unlike its antecedent, open courseware usually written materials or videotapes

of lectures that make you feel as if youre spying on a class from the back of the room the

MOOC is a full course made with you in mind.

The medium is still the lecture. Thanks to Khan Academys free archive of snappy instructional

videos, MOOC makers have gotten the memo on the benefit of brevity: 8 to 12 minutes is

typical. Then this is key videos pause perhaps twice for a quiz to make sure you

understand the material or, in computer programming, to let you write code. Feedback is

electronic. Teaching assistants may monitor discussion boards. There may be homework and a

final exam.

The MOOC certainly presents challenges. Can learning be scaled up this much? Grading is

imperfect, especially for nontechnical subjects. Cheating is a reality. We found groups of 20

people in a course submitting identical homework, says David Patterson, a professor at the

University of California, Berkeley, who teaches software engineering, in a tone of disbelief at

such blatant copying; Udacity and edX now offer proctored exams.

Some students are also ill prepared for the university-level work. And few stick with it. Signing

up for a class is a lightweight process, says Dr. Ng. It might take just five minutes, assuming

you spend two devising a stylish user name. Only 46,000 attempted the first assignment in Dr.

Ngs course on machine learning last fall. In the end, he says, 13,000 completed the class and

earned a certificate from him, not Stanford.

Thats still a lot of students. The shimmery hope is that free courses can bring the best

education in the world to the most remote corners of the planet, help people in their careers,

and expand intellectual and personal networks. Three-quarters of those who took Dr.

Pattersons Software as a Service last winter on Coursera (its now on edX) were from outside

last spring by Dr. Agarwal. But both attracted highly educated students and both reported that

over 70 percent had degrees (more than a third had graduate degrees). And in a vote of

confidence in the form, students in both overwhelmingly endorsed the quality of the course: 63

percent who completed Dr. Agarwals course as well as a similar one on campus found the

MOOC better; 36 percent found it comparable; 1 percent, worse.

Ray Schroeder, director of the Center for Online Learning, Research and Service at the

University of Illinois, Springfield, says three things matter most in online learning: quality of

material covered, engagement of the teacher and interaction among students. The first doesnt

seem to be an issue most professors come from elite campuses, and so far most MOOCs are

in technical subjects like computer science and math, with straightforward content. But

providing instructor connection and feedback, including student interactions, is trickier.

Whats frustrating in a MOOC is the instructor is not as available because there are tens of

thousands of others in the class, Dr. Schroeder says. How do you make the massive feel

intimate?

Thats what everyone is trying to figure out.

Many places offer MOOCs, and more will. But Coursera, Udacity and edX are defining the form

as they develop their brands.

THE FLAVOR OF THE MOOC

Coursera casts itself as a hub Dr. Ngs word for learning and networking. The learning

comes gratis from an impressive roster of elites offering a wide range of courses, from computer

science to philosophy to medicine. Not all are highbrow or technical; Listening to World Music

from the University of Pennsylvania aims to broaden your iPod playlist.

While Coursera will make suggestions, Dr. Ng says, ultimately all pedagogical decisions are

made by the universities. Most offerings are adapted from existing courses: a Princeton

Coursera course is a Princeton course. But the vibe is decidedly Facebook build a profile,

upload your photo with tools for students to plan meet-ups with Courserians in about

1,400 cities worldwide. These gatherings may be bona fide study groups or social sessions.

Membership may be many or sparse.

No one showed at the meet-up that Stacey Brown, an information technology manager at a

Hartford insurance company, scheduled for a 14th-floor conference room on a Thursday after

work, despite R.S.V.P.s from a few classmates in the area. Hes taking three Coursera MOOCs,
